No additional AE/No PQC; Medical assistant reports AE for PROQUAD. A 25-year-old patient came into the practice requesting an "MMR" vaccine. The patient was inadvertently administered PROQUAD. Childhood vaccination history for this patient is unknown; however, HCP indicated; This spontaneous report was received from a medical assistant and refers to a 25-year-old female patient. The patient's medical history included chicken pox (in 2022) (reported as "Childhood vaccination history for this patient was unknown; however, the reporter indicated the patient was seen in 2022 and had chicken pox"). The patient's concurrent conditions and concomitant therapies were not reported. On an unknown date, the patient was vaccinated with a dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), lot #Z015271, expiration date: 30-Mar-2027, 0.5ml) (anatomical location and administration route were not reported); diluted with a sterile diluent (Hollister-Stier STERILE DILUENT), (anatomical location, administration route, lot # and expiration date were not reported) as prevention. The reporting medical assistant stated that the patient was into the practice requesting an "MMR" vaccine; however, the patient was inadvertently administered Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), instead. The reporter believed this could have been her first "varicella" vaccination but could not confirm. No further medical history was known or provided, and no additional adverse events were reported.
