Symptom narrative

Patient here for nurse visit for vaccination, 2nd HPV shot. MA administered HPV vaccination. MA turned back towards to the computer, saw patient lean forward, MA reached for her, patient proceeded to fall and was assisted to ground by MA and father. Patient hit right knee on bed. MA asked for help, PA came and assessed patient. Patient woke up within 20 seconds, vitals obtained. Abrasion found on knee and bandaged per PA. Given water, apple juice,and crackers continued to monitor vitals to ensure stability. Patient reported that she had not eaten prior to appointment. Dr was notified. After approximately 15 minutes with stable vitals patient was released to home.
