Site: Pain at Injection Site-Mild, Additional Details: Vaccine was not reconstituted. Only diluent was administered. Patient was contacted and informed. Patient said she will contact her doctor and call us back. We advised that she needs to retake the vaccine because she did not receive it. Only sterile water was administered.
