Systemic: gave shot 1 month apart . mistakenly gave a second dose-Mild, Additional Details: relaized patient had rec. tdap on 4/21/2026. i changed date because 1 saw unknown on sheet. originally thought last admin date 2015. I misread vaccine report. i did not see the 4/15/2026 date nor did insurance reject. I did sign sheet for 5/21/26 and was a little confuesd to the word unknown. pt rec. 2nd dose on 5/21/26. I called patient to inform. no answer. i will continue to try reach pt.
