Symptom narrative

Site: Pain at Injection Site-Mild, Site: Shoulder Joint Injury (prolonged pain, tingling, etc.)-Mild, Systemic: Tingling (specify: facial area, extemities)-Mild, Additional Details: The patient returned and expressed concern that the shingle vaccine may have been adminstered too high in the arm. During the consultation, the patient reported pain, tingling, and numbness radiate down the arm, as well as pain near the clavicle.The patient was advised to seek medical evaluation if symptoms persist, worsen, or fail to improve.
