Symptom narrative

Patient was brought into the office for vaccine update. The record, and the immunization record given to us by pt guardian did not reflect the above imms. and record forecasted he was due. They were given in the office. Patient stayed 15 minutes after administered without incident/complaints. The patient's guardian called the office the next day to let us know they had another vaccine record, and received duplicate vaccines.
