Caller corrected temperature excursion information provided on previous call and was able to provide patient information for administered products. The patient's medical history, concurrent conditions and concomitant medications were not reported. On 11-May-2026, the patient was vaccinated with and improperly stored dose of Hepatitis A Vaccine, Inactivated, Adult, 50 u/mL(VAQTA) 0.5 mL (total), Batch/Lot Number: Z014461 has been verified to be a valid batch/lot number for Hepatitis A Vaccine, Inactivated with expiration date: 23-Apr-2027 as prophylaxis (anatomical location and route of administration was not reported). The vaccine was stored at a temperature of 0.97 C degrees for 27 hours and 30 minutes (Improper storage of product in use). There were no previous temperature excursions.
