only the diluent administered; only the diluent administered; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 64-year-old male patient who received Herpes zoster (Shingrix) solution for injection (batch number FX753 with expiry date 07-JUL-2027) for prophylaxis. On 19-MAY-2026, the patient received the 2nd dose of Shingrix. On 19-MAY-2026, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: only the diluent administered) and inappropriate dose of vaccine administered (Verbatim: only the diluent administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
