only the diluent administered; only the diluent administered; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 64-year-old male patient who received Herpes zoster (Shingrix) solution for injection (batch number FX753 with expiry date 07-JUL-2027) for prophylaxis. On 19-MAY-2026, the patient received the 2nd dose of Shingrix. On 19-MAY-2026, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: only the diluent administered) and inappropriate dose of vaccine administered (Verbatim: only the diluent administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK receipt date: 21-MAY-2026 The health care professional wanted to determine whether they could get the patient in right away to restart the vaccine, as they had someone who had been given only the Shingrix diluent and it was asked when the vaccine could be readministered.
