Symptom narrative

Product needle that is provided to draw up the vaccine after it is mixed has been changed to a smaller needle that is not adequate for use to draw up vaccine before it is administered to the patient. It is flimsy and broke off while trying to draw up the vaccine. When the provided needle broke off, it caused an accidental needle stick injury to our provider administering the vaccine.
