Phone Note Initial Intake: Concerns/Comments: Pt. mom called to report that her son had an adverse reaction to vaccines on at his WCC. He was not asked to wait 15 min after the vaccine was given, and when he left he passed out in the elevator. He is doing fine now and did not have any other reaction. I called (mom) back to see how pt is doing and she reported he is doing fine. I did tell her that since he has had an adverse reaction, we will want him to stay for 30 minutes after his next vaccine.
