Symptom narrative

Patient felt dizzy immediately following vaccinations. After two minutes, she felt like she was going to pass out and fainted for about 10 seconds. Patient was sitting in chair when she passed out and remained in the chair. Patient came to, was given water and an ice pack. Patient felt nauseous but did not vomit. She sat for about 15 minutes until she felt steadier. Patient was evaluated by RN. Patient was cleared to g o home with her mother.
