Symptom narrative

NO ACTUAL ADVERSE EVENT. THE VACCINE IS A COMBINATION, 2- COMPONENT VACCINE.; 1) PREFILLED SYRINGE, CONTAINING MEN B COMPONENT and 2) vial of MCV4 component. they are supposed to be mixed together in syringe before administration. Nurse incorrectly administered only the Men B component in the syringe, and did not mix it with MCV4 component in the vial. Pt received an incomplete vaccine- only Men B was administered

EGGS BY THEM SELVES; causes nausea only, CAN TOLERATE EGGS COOKED WITH OTHER FOODS
