Symptom narrative

Patient given Pediarix. Patient has been getting only DTAP and HIB shot since birth. Parents only wanted to start with a couple vaccines (slow schedule). Nurse/Dr ordered Patient the Pediarix vaccine which is a combo shot that has polio dtap and hep b vaccine. They did not confirm this combo shot or ask if this is what we wanted and they just gave the patient it.
