Symptom narrative

Immediately after the syringe was withdrawn from the patient, patient stated "I feel lightheaded", and closed his eyes. Patient fainted for about 30 seconds. He then opened his eyes and asked what happened. Informed him he fainted. He was laughing and said that's the second time that happened. Then his father explained he meant he fainted before after a nosebleed (not vaccine related). Patient was able to communicate with nurse and parent. Patient was observed for 30 minutes with no other reaction noted. Patient left with father in stable condition. Father is taking him to get something to eat next door. Vital sign at time of discharge: BP 93/60 P 84 T97.5 O2 98 R 20
