Symptom narrative

No symptoms were reported; Customer reported that extra dose of VARIVAX was inadvertently administered to 17 years old patient; This spontaneous report was received from a physician Assistant and refers to a 17-year-old patient of unknown gender. Information regarding the patient's concurrent conditions, medical history, or concomitant medications was not provided. On 27-May-2026, the patient was inadvertently vaccinated with an extra dose of Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) 0.5 ml (route of administration and anatomical location not reported, lot number Z016560, which was verified to be valid, expiration date established on 28-Oct-2027), the suspect vaccine were reconstituted with sterile diluent (Hollister-Stier) (lot number Z004374, which was valid, expiration date established on 12-Dec-2027) for prophylaxis. No symptoms were reported (No adverse event).
