Symptom narrative

Colitis flare with diarrhea beginning 3 or 4 hours after vaccination . Became severe so seen in an ER for rehydration 2 days after vaccination. Became uncontrollable diarrhea leading to hospitalization 5 days after vaccination with diagnoses of acute diarrhea, dehydration with secondary acute kidney injury

Multiple sclerosis, collagenous colitis, hypothyroidism, androgenetic alopecia

Levothyroxine, Budesonide, minoxidil, spironolactone ,estradiol, progesterone

Allergies

Sulfa, Cipro, arnica

Blood chemistry panel was unremarkable except for elevated creatinine, stool tested negative for multiple infectious pathogens, abdominal CT scan was negative
