a patient was administered only the Prefilled Syringe of the Liquid MenB Component of Penmenvy on 26May2026 as the first dose in the series; a patient was administered only the Prefilled Syringe of the Liquid MenB Component of Penmenvy on 26May2026 as the first dose in the series; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 16-year-old male patient who received Men ABCWY NVS (Penmenvy) suspension for injection in pre-filled syringe (batch number 3S9XZ with expiry date 23-MAR-2027) for prophylaxis. Previously administered products included MenQuadfil (received in JUL-2022). On 26-MAY-2026, the patient received the 1st dose of Penmenvy. On 26-MAY-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: a patient was administered only the Prefilled Syringe of the Liquid MenB Component of Penmenvy on 26May2026 as the first dose in the series) and inappropriate dose of vaccine administered (Verbatim: a patient was administered only the Prefilled Syringe of the Liquid MenB Component of Penmenvy on 26May2026 as the first dose in the series).The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt date:27-MAY-2026 The reporter supplied as one vial of lyophilized MenACWY component and one prefilled syringe of liquid MenB component which must be combined before administration. The patient was administered only the Prefilled Syringe of the Liquid MenB Component of Penmenvy. The registered nurse mentioned the patient previously received a dose of MenQuadfi.
