Patient received vaccine. Within 2-5 minutes of patient receiving vaccine, started not feeling well. Patient stated he felt nauseous so decided to walk to the bathroom. By time patient walked to the bathroom, patient stated he didn't feel well and felt like he was going to pass out. Patient appeared pale in color, sitting on bathroom floor. Alert. Did appear shaky. Patient was given apple juice. Thought to be feeling better so moved to chair. Upon getting into chair, 1st blood pressure was 70/31, second blood pressure read 75/35. Patient then went limp, so was lowered to the flood by mother and LPN. LOC less than 1 minute. Patient then was lying on the floor with feet elevated. BP started to rise with one reading being 118/73. Patient alert at the time. Patient was sweating as well as still shaking. Decision was made to call 911 due to patient presentation. Upon arrival of EMS, patient stating he was starting to feel better with better color and BP being 106/66. Patient was still sweating, shaking, and stated he felt tired. EMS transported patient to local ER.
