nurse gave Penmenvy without mixing. So, patient only got the MenB portion; nurse gave Penmenvy without mixing. So, patient only got the MenB portion; This non-serious case was reported by a nurse via sales rep and described the occurrence of inappropriate preparation of medication in a 16-year-old male patient who received Men ABCWY NVS (Penmenvy) suspension for injection in pre-filled syringe for prophylaxis. On 21-MAY-2026, the patient received the 1st dose of Penmenvy (left deltoid). On 21-MAY-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: nurse gave Penmenvy without mixing. So, patient only got the MenB portion) and inappropriate dose of vaccine administered (Verbatim: nurse gave Penmenvy without mixing. So, patient only got the MenB portion). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
