After receiving YF vaccine client almost immediately complained of itching in the area of her right arm where vaccine was given. On observation, there was a blanching red rash/welt. Bandaid removed, patient was observed for 30 minutes post administration. Itching/dermatitis also present around bandaids on left/right deltoid, which were both removed. Pt states she had a similar reaction to the Tdap vaccine a few months ago and denies anaphylaxis. Client denies: SOB, swelling of face or throat, GI symptoms, or any other symptoms other than itching in her arms in vicinity of injection sites, and a new onset headache. Spoke with (name withheld) on RN hotline who agreed with plan for patient to manage at home (she is being driven home by partner) with topical hydrocortisone and benadryl, and to watch for signs of anaphylaxis and get emergency treatment if those should come about. Patient states she agrees to this plan and will call with questions/concerns.
