Symptom narrative

My 4-year-old son, received a DTaP/IPV combination vaccine at his pediatrician?s office on Friday 5/29/26. The injection was administered at an incorrect intramuscular site ? too low on the arm, below the deltoid resulting in what appears to be subcutaneous tissue injury. He is experiencing extreme pain, significant swelling (2inchx3inch), and redness well below the injection site expanding to his elbow consistent with subcutaneous injection of the vaccine. When I reported the reaction with photos via the patient portal, the on-call physician responded dismissively, stating the Band-Aid marks (in the photo) appeared correct and that the location of the reaction did not indicate incorrect administration even though the photo clearly displays the injection site far below where standard deltoid injections are administered. As a registered nurse with 10 years of emergency medicine and clinical experience, I believe this response is both clinically inaccurate and potentially constitutes concealment of a known administration error.
