Symptom narrative

Patient presented to the clinic with complaints of his left arm being red and warm to the touch and spreading. Patient had vaccines done at PCP office on 5/29/26 (3 days ago) and started noticing a red spot on his left arm yesterday. The red area is on his left arm and starts about 4 in below the acronym process and extends 9.5cm long towards his elbow and is 11.5cm wide extending around his arm. All three vaccines were given in the same arm by an MA at the PCP office and closer than one inch together and in the sub q area.

Fluticasone PRN, Ventolin PRN, Loratadine PRN, dextroamphetamine-amphetamine ER 15mg
