Patient came into the office on 05/29/2026. Patient was supposed to receive an HPV vaccine but instead received a Prevnar 20 vaccine. I called Mother of child yesterday and she reported patient went to Urgent care over the weekend due to a reaction to the Prevnar 20 vaccine. Arm was swollen and red. Patient also had a sore throat.
None at the time of vaccination. A month prior possible UTI
Multivitamin
No known allergies
I asked mother of patient what was done at urgent care for the reaction to the vaccine and she stated they gave the patient Tylenol and Advil and advised follow up at our office.
