After receiving the meningitis vaccine, the patient reported feeling lightheaded and experienced a brief syncopal episode lasting approximately 1 minute. Water had been provided prior to the event. A code white was initiated and 911 was contacted per protocol. The patient regained consciousness without complication and returned to baseline after resting for approximately 10 minutes. The patient stated that he had been feeling somewhat anxious and nervous prior to vaccination, however this information was not disclosed to staff before administration of the vaccine. During the observation period, the patient was alert, oriented, and conversing appropriately about his upcoming college plans, expressing excitement about attending school. Although emergency services were contacted, the patient's mother declined EMS transport and elected to take the patient to urgent care for further evaluation, as this was the first time the patient had experienced such an event. The patient and mother were advised to inform healthcare personnel of this experience, including any history of anxiety, nervousness, lightheadedness, or syncope associated with vaccinations, prior to receiving future immunizations so that appropriate precautions and monitoring can be provided. The patient was continuously monitored, and I remained at the patient's side throughout the observation period until he was stable and departed the store with his mother.
