Symptom narrative

Notable swelling of the left shoulder and supraclavicular region. Palpable subclavicular mass consistent with a lymph node; mobile and tender to palpation. No cervical lymph nodes enlarged. Swelling is non-pitting and significant in size. Injection site is swollen without erythema or increased warmth. Advised that these reactions are likely normal side effects and to try tylenol/ibuprofen, and rest. Reporting to VAERS as precaution.

Diabetes type 2, hyperlipemia

Other medications

Atorvastatin, Fenofibrate, Jardiance, and Metformin. Denies tylenol or ibuprofen use.

Allergy to Codeine
