Symptom narrative

Approximately two hours following the shot, patient experienced fatigue followed by 10 minute long: loss of hearing, ear ringing, patchiness of sight. Five minute loss of sight. One minute loss of consciousness. 24 hour headache, 24 hour arm ache.
