pt received the HPV9 vaccine and a finger poke for a lipid panel at the end of the visit. Shortly after the CMA leaving the room the pt's father reports that he was looking at his phone when her heard a bump on the wall. He looked up as the patient was sliding against the wall to the left on the table. Dad called for help. LPN went in the room where dad had already laid the patient down on the exam table.
