Symptom narrative

After playing baseball today, he developed an itchy rash that progressively worsened and became diffuse. While en route to the ED, he had mild shortness of breath that improved by arrival. States that his shortness of breath is completely resolved. He denies lip, tongue, or throat swelling and denies throat tightness. He has nausea with abdominal cramping pain and mild lightheadedness. He denies syncope or vomiting. Mother states that he received a meningitis vaccine yesterday in the left arm and this was his second dose; he has not had prior vaccine reactions. He denies new exposures around the baseball field and denies new foods or medications.
