Symptom narrative

Pt and mother went out to the lobby. Pt passed out falling backwards hitting the back of her head on the floor. Pt was assisted to a wheelchair and taken to a room. Pt did lay down on the bench with her head in mother's lap. Some vomiting initially. Provider was notified and came to the room to evaluate patient. Pt's mother reported that she had not has anything to eat or drink since dinner the night before. Vitals were taken multiple times. Pt attempted to sit up once after being taken to the room then laid back down after a couple of minutes. Patient vomited a small amount a couple of times. Complained of dizziness, nausea. BP stable. MD in room several times between seeing patients. Pt attempted to sit up about 11:50. Patient wasn't even upright and went right back down to the bench. Patient began to vomit again. Provider in room to evaluate again and advised mother she would need to go to the hospital by ambulance. 911 called for transport to hospital.

Basic Metabolic Panel and CBC done in the ED. Also a CT w/out contrast done.
