Was Given Engerix-B As The Second Dose Instead of Heplisav-B; Initial report received on 20-May-2026. A consumer reported that she, a 78-year-old female (gender, race, ethnicity not reported) received ENGERIX-B (hepatitis B vaccine) for hepatitis B immunization instead of HEPLISAV-B. No medical history or concomitant medications were reported. On an unknown date, the patient received an unknown dose of a hepatitis B vaccine (ENGERIX-B (lot #, route, and site not reported). On an unknown date, the patient received dose 1 of HEPLISAV-B (lot#, expiration date, NDC #, route and site not reported). ENGERIX-B was given instead of dose 2 of HEPLISAV-B.
