Symptom narrative

no additional AE; MMR-II and RECOMBIVAX HB were administered on the same date to a patient after a TE excursion; This Spontaneous report was received from a registered nurse and refers to a patient of unknown age and gender. The patient's medical history, past medications, and concomitant medications were not reported. On 07-May-2026, the patient was vaccinated with improperly stored dose of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live powder and solvent for solution for injection (M-M-R II) (Batch/Lot Number: Z006283, expiration date: 19-Mar-2027) reconstituted with sterile diluent (lot # not provided), and hepatitis B vaccine (Recombinant) (RECOMBIVAX HB), 5 mcg/0.5mL Suspension for injection 64457246:4093 (Batch/Lot Number: Z010142, expiration date: 09-Feb-2028) (improper storage of product in use). The vaccines underwent a temperature excursion of 65 Fahrenheit for 6 hours. There was no previous temperature excursion. No additional adverse event was reported. Lot # is being requested and will be submitted if received.
