Symptom narrative

Patient had labwork + vaccines (HPV and Hep A) performed on 6/2/26. Post-vaccination, patient was seated on the examination table talking with the nurse and father about the meningococcal B vaccine and experienced syncope. Patient fell forward off the examination table and onto the floor. Per patients father, patient hit his head. Patient was advised to prop his knees up and stay on the floor while nurse retrieved an additional provider. NP performed a physical examination and neuro check. With no concerns, patient was moved to a sitting position. Nurse obtained vital signs - bp, pulse, O2. Patient was given crackers and juice while sitting. Patient waited 15 - 20 minutes before leaving. Patient and father education on signs/symptoms to monitor for and to seek help if they arise.
