Erroneous encounter patient was given wrong discharge paperwork with correct first name wrong last name. D/T patients' health issues she could not verify identity and family member left patient at the injection area. RSV vaccine was given twice to this patient first dose 4/29/26 another 6/2/2026. Once error was noticed department supervisor notified.

Abdominal issues, Anemia, small intestine gastritis, internal hemorrhoid, colon polyp, diverticulosis of colon, spinal stenosis, osteoarthritis of left knee
