Moderna Spikevax administered to patient past by use date. Vaccine received by facility and placed in vaccine refrigerator on 01/15/2026. This created a by use date of 03/16/2026 as indicated by the vaccine administration instructions. However the receiving facility did not correctly mark this date and administered the vaccine based off the expiration date on the box rather than the by use date. Patient guardian was notified of error and no further adverse events or reactions in patient were reported.
