Moderna Spikevax vaccine administered past by use date. Vaccine was received and placed in vaccine refrigerator on 01/15/2026. In accordance to vaccine administration guidelines, this created a by use date of 3/16/2026. However this was not properly marked by the facility and vaccine was administered on 04/09/2026 in compliance with printed expiration date but not by use date. Patient guardian was notified of the error and reported no other adverse events or reactions in patient.
