pt called to ask what was the date she received her flu shot. she stated she had a tingling sensation in her arm and it went up her shoulder to her neck. She did not seek medical treatment until several month later (IMZ date 11/15/25) she recently saw phyician and they believe it was nerve related pain with possible nerve damage. she did not seek any financial compensation today .
