Symptom narrative

Nurse accidentally administered expired vaccine. The vaccine had expired at midnight the day before. Immunization Nurse was notified and instructed to re-vaccinate per CDC guidelines. Mom and patient aware of incident. Dr. was notified. Pt. was re-vaccinated on the same day as expired dose was given. Nurse will contact patient/parent to assess for any adverse reaction.
