Symptom narrative

Patient came in for the second Covid vaccine. I placed it IM in LT @4:15pm. I noticed the vaccine expired on 6/1/26 after I had administered it. The patient was monitored in the clinic for 15 minutes, and no adverse effects were observed. The patient waited in the clinic after the 15 minutes were up so the provider could discuss the incident. No adverse effects were observed after this time.
