Symptom narrative

On 6-2-26 the patient came to the pharmacy to get her second Gardasil vaccination. At this time it was brought to my attention, Pharm. D Pharmacist Manager, that the patient was outside of the recommended age to receive this vaccine. The patient is 53 years old, was 52 when the vaccine was administered. The insurance company informed us that in order to give the vaccine we would have to acknowledge that the patient was outside of the range and put in an override code. It was my professional judgement that this vaccine did not need to be administered to the patient. I explained this to the patient and she left the pharmacy. The patient was concerned about potential side effects and I informed her that there should be no side effects, but that I would enter this into the VAERS database since she did not complete the series as recommended by the CDC guidelines. The patient was satisfied with the outcome.
