Symptom narrative

Had to go to urgent care due to severe chills, then waking up in the wee hours with unending racing heartbeat, coughing and shortness of breath.

Current illness

Only allergies.

Medical history

Interstitial Cystitis, eczema around eye area, hypothyroidism.

Other medications

Levothyroxine daily for hypothyroidism. Topical estrogen 2x/wk. Protopic ointment as needed for eczema. Seed probiotic and a multivitamin daily.

Allergies

Dilaudid, Erythomycin, Penicillin, Singular. Must avoid garlic, onion, citrus, chocolate, tomato, caffeine and other certain foods due to FODMAP sensitivity and triggers as an IC patient.

Lab data

EKG, blood work, chest x-ray, and CT scan with contrast.

Prior vaccinations

Flu vaccine at least a few years ago, and COVID Moderna vaccine on 11/7/21.
