Symptom narrative

Patient, 12 year old, received HPV#1. Directly after receiving the vaccine, while in the chair, the patient's body started to convulse and he became unresponsive briefly. The patient was lowered to the ground and head was protected. The patient opened his eyes as he was being lowered to the ground, and stated his stomach hurt. Patient became diaphoretic. over a period of approx. 5 minutes while on the ground patient had symptoms of syncope but never became fully unconsciousness. Code blue was called, and patient was brought to the emergency room for further evaluation.

6/5/2026 ECG Normal sinus rhythm with occasional PVCs, rate 56 bpm, normal axis. There is no ST segment elevation or depression and no T wave inversion. 6/5/2026- Glucose 120.
