History of tethered cord with surgical revision at 3 yo.Patient was given Proquad vaccine in left thigh today after administration of Kinrix in right thigh by nurse in office. Pt intially cried but tolerated well . PT was consolable by mom . Family brought in a cat to console pt and brought it to her face but unclear if pt was holding cat as staff left the room . As pt was in lobby , starting sneezing and coughing and crying. Pt brought back to the room. Where aunt and mom were reporting pt had lip swelling and pt then started vomiting profusely. Pt only had milk for breakfast that morning. I was called into the room and noted pt to have slight lower lip swelling, no tongue swelling and pt vomiting into emesis bag. pt had been noted to have slightly dusky lips but it seemed to go away. No wheezing. Decision was made to give pt 0.15mL Epipen Jr. which was injected into left thigh. EMS was called and responded quickly. Pt's limbs were cold and clammy. Her pulse ox was 96% but when EMS arrived it went down to 91% and O2 give. Pt slightly tachycardic after Epi pen administration, vomting stopped and then pt had anther small episode. Pt was alert , awake , and oriented this entire time. Pt was taken to Er for further evaluation and management.
