Patient received the MCV4 (MenQuadfi) vaccine with sterile diluent on 06/03/02026 at 9:59am. Vaccine was administered IM in the left deltoid. Vaccine dose not valid per CDC and manufacturer. MCV4 (MenQuadfi) Manufacturer: Sanofi Pasteur NDC# 49281-590-10 Lot # U8697BA Exp Date: 4/30/2029 Onsite provider (name withheld of MD) notified patient and guardian (mother) and answered any questions and concerns. The patient's PCP (name withheld of APRN, PNP) was notified of the event and notified to reach out to family relaying that the vaccine dose was not valid.
