16 Year Old Received A Dose of Heplisav -B; Initial report received on 22-May-2026. A pharmacist reported that a 16-year-old (gender, race, ethnicity not provided) received HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization. No medical history or concomitant medications were reported. On 22-May-2026, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot #, expiration date, NDC #, site, and route not reported)]. The patient was 16 years old at the time of vaccination. No other information was reported.
