Patient filled out consent form for Hepatitis A, but we processed and gave Heplisav-B (Hepatitis B.) Patient got a reminder for her second injection then we realized the mistake. Patient was okay with the mistake. She will be in to get the 2nd hepatitis B and hepatitis A vaccines.
