On 01-JUN-2026, the patient received Priorix (subcutaneous). On 01-JUN-2026, an unknown time after receiving Priorix, the patient experienced inappropriate preparation of medication (Verbatim: maladministered the Priorix diluent only (without reconstituting the dose) to a patient) and inappropriate dose of vaccine administered (Verbatim: maladministered the Priorix diluent only (without reconstituting the dose) to a patient.).
