maladministered the Priorix diluent only (without reconstituting the dose) to a patient; maladministered the Priorix diluent only (without reconstituting the dose) to a patient.; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 8-month-old male patient who received MMR (Priorix) solution for injection in pre-filled syringe (batch number T543P with expiry date 19-MAR-2027) for prophylaxis. On 01-JUN-2026, the patient received Priorix (subcutaneous). On 01-JUN-2026, an unknown time after receiving Priorix, the patient experienced inappropriate preparation of medication (Verbatim: maladministered the Priorix diluent only (without reconstituting the dose) to a patient) and inappropriate dose of vaccine administered (Verbatim: maladministered the Priorix diluent only (without reconstituting the dose) to a patient.). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
