Site: Pain at Injection Site-Mild, Site: Redness at Injection Site-Mild, Systemic: Has about 3 in square that was at/ surrounding injection site and warm to touch.-Mild, Additional Details: Patient stated no rash or swelling, just the red square that is warm to touch with some soreness. Was instructed by DR that she was experiencing a reaction to immunization and to inform pharmacy.
