Symptom narrative

Site: Pain at Injection Site-Mild, Additional Details: Gave a second dose of the covid vaccine within the 48-week period, basing it off of old guidelines since it's now 1 dose no matter the history. Called and talked to patient, going over concerns. He hasn't reported any adverse effects yet, just choosing mild arm pain to submit, but wanted to document the error in giving the vaccine early.
