Site: Pain at Injection Site-Mild, Site: Redness at Injection Site-Mild, Systemic: Chest Tightness / Heaviness / Pain-Medium, Systemic: Dizziness / Lightheadness-Medium, Systemic: Exhaustion / Lethargy-Medium, Systemic: Fever-Medium, Systemic: Headache-Medium, Systemic: Nausea-Medium, Systemic: Weakness-Medium, Additional Details: Patient received 3 vaccines on 5/31/26 (Boostrix, Capvaxive, Shingrix). Pt called pharmacy on 6/3/26 reporting adverse reaction. Pt believes reaction was caused by the shingles vaccine. Reported headache, fever, arm soreness, nausea, dizziness, fatigue, and shortness of breath after vaccinations. On 6/3 also reports a burning rash at injection site. Pt treated pain and fever with ibuprofen and tylenol. Plans to treat developed rash with benadryl. Advised urgent care if SOB does not resolve.
