Symptom narrative

Site: Swelling at Injection Site-Mild, Additional Details: excessive bleed at injection site and lump about half inch in size (blood pooling under skin) formed immediately. Bleeding stopped but bump remained. Informed patient to monitor bump and if it gets bigger to seek medical assistance. Patient reported no pain. Patient came in next day and still reports no pain but bump remains.
